How do I fill out a Neurotoxicity Assessment for an additional friend/family member?

This is the link that you'll use if you have purchased an additional consultation for a friend or family member: https://apply.pompaprogram.com/register-secondary. This should allow you to create two accounts for your Neurotoxicity Assessments. Each person will still need to create their own account using this link. Once you've created the account for the first person, they will then fill out the assessment in its entirety. Once you have opened the page and began to fill out the assessment, do not navigate away from the page until the entire assessment has been filled out and submitted. You should put aside about 30 minutes for each assessment that needs to be completed.
